If this is your first visit/Initial Evaluation at SPTA, or if it has been more then 60 days since your last visit:
Please bring your insurance card, copay(if your plan requires one) and a Physical Therapy prescription from your doctor.
You have the option to download our forms and complete them before you arrive for your first appointment OR please arrive 10-15 minutes early to fill out this paperwork in our office. All patients are required to finish the complete set of paperwork before your first appointment.
Please dress appropriately. For example, if you are being seen for a knee injury, please wear shorts; if you are been seen for a neck or shoulder injury, please wear a tank top. All Sports and PT clinics have private rooms where you will be able to change your clothes, if needed.
