Your First Visit

Please bring your insurance card and prescription written by your doctor.

Please download our forms and have them completed when you arrive for your first appointment OR please arrive 10-15 minutes early to fill out this paperwork in our office. It is required to complete all paperwork before your first visit.

Please dress appropriately. If you are being seen for a knee injury, please wear or bring shorts; if you are been seen for a neck or shoulder injury, please wear a tank top.
